*"I'm getting handfuls in the shower."* *"My parting's getting wider."* *"My ponytail's the size of a pencil."* I hear those three sentences in my clinic every week. So before anything else: **you are not imagining it, and you are not being vain.** If a GP or a hairdresser told you it's "just your age"  I used to say that too. I was wrong. Here are the five things I now explain to every woman in that chair.

A patient I'll call Helen, 52, brought a sandwich bag of hair to her appointment. One week of showers. Her hairdresser said she was "over-washing". She wasn't. Oestrogen has one quiet job on your scalp: **it keeps follicles in the growing phase for longer.** The follicle has its own oestrogen receptors (Ohnemus et al., *Endocrine Reviews*, 2006). When the menopause hits and oestrogen falls, follicles drop out of growth early — that's the shedding. Then each one restarts slightly smaller. Finer, shorter hair. Perimenopausal periods drain iron faster than you replace it. **Only TSH checked.** The standard panel misses about **1 in 10** thyroid problems, more so in women (Canaris et al., *Arch Intern Med*, 2000). Straw-like hair and thinning outer eyebrows are the tell. **Waking at 3 am.** Broken sleep keeps cortisol high, and cortisol pushes follicles into rest en masse — telogen effluvium. Ask your GP for ferritin and a full thyroid panel. Say the words "hair loss". It's free. *But fixing that takes months. So in the meantime, women buy things.*

The hair you can touch is dead keratin. Nothing you put on it changes what the follicle does next. That one fact explains most of the disappointment in my clinic. **Thickening shampoos** coat the strand for a wash or two. **Biotin** only helps if you're deficient, which is rare (Patel et al., 2017). A popular way to spend £12 at Boots. **Rosemary oil** has one small 2015 study. In men. **Oral iron** helps a low ferritin — slowly, and it does nothing for the oestrogen problem. None of these are scams. They're aimed at the wrong place. **You've been buying for the hair. The follicle is what's failing.** *So: can a quiet follicle be told to start again?*

A miniaturised follicle isn't dead. It's dormant. And dormant cells can still receive instructions. Two ingredients do exactly that. **Peptides** — short amino-acid chains that act as messengers.
